Take a look at this quick tutorial I threw together to show how to leverage the responses from a Google Form. Since Google Forms can be accessed from nearly any device with internet access, we've adopted a simple evaluation tool that our student supervisors fill out to evaluate their peers. The data from those submissions help provide us with real-time results and answers.
In this lengthy tutorial, we take a look at how to automate form responses to fill-out a pre-fabricated template and email a PDF version.
Recommend to watch the video then look at the sample folder I created. Since the folder can be viewed by anyone with the link, I recommend you create your own test forms in a separate folder on your Google Drive.

Due to the large interest and complexity of the conversation, I'm offering a few, free webinars throughout the Summer on the topic. The webinars are through Google Hangouts on Air (HOA) (essentially, LIVE YouTube). You will not need a Google + account to view, but it does allow you to be more interactive with the presentation (i.e. ask questions / leave comments).
The first HOA on the series will be June 3 at 11:30am Eastern. It is expected to last about an hour and a half. HOAs are come and go events. If you cannot make that date and time, the conversation will be recorded and posted to YouTube for future watching.
